Once submitted, a member of our team will be in touch with you. Becoming a hospital volunteer involves several steps, and we will guide you through the process.
Here are the requirements for volunteering:
1. You must be 15years of age or older.
2. We are unable to accommodate court-ordered community service.
3. If you're interested in short-term opportunities, such as summer-only positions, please note that the application process and onboarding can take up to 3-4 weeks.
4. To maintain active volunteer status, we require a minimum of 50 hours of service per year, unless there are exceptional circumstances. We'll provide individual support for such situations.

I understand that if accepted as a volunteer, it is my responsibility to read the rules and regulations for student volunteers,to be prompt, to call if I am unable to report as assigned,and to perform my assigned duties to the best of my ability. I also understand that I have to go through a health screening and provide evidence of having immunizations. I agree to the 50 hour minimum required to be a volunteer at Froedtert Holy Family Memorial Hospital, and am aware that no community service hours will be signed for unless I have completed this 50 hour minimum. The information in this application is true to the best of my knowledge. An incomplete answer or false answer to any question on this form may be grounds for not being able to volunteer or for my dismissal after I have begun my volunteer service. I understand the assignment I accept is strictly voluntary and I will not receive any monetary compensation from Froedtert & the Medical College of Wisconsin, Froedtert Holy Family Memorial or any of the associated physicians, employees and/or patients for carrying out the duties of my volunteer assignments. I understand that as a volunteer I am required to complete annual education and receive the flu vaccination (unless proper exemption provided),and abide by all compliance requirements in order to volunteer. I understand that this agreement allows the Froedtert & the Medical College of Wisconsin Froedtert Holy Family Memorial Hospital Volunteer Services Department to continue the volunteer application and onboarding process, such as requesting information as necessary to verify my application and gives permission to conduct a background information disclosure check. If under the age of 18, I can confirm that I have received permission from my parent/legal guardian to submit this application.
